As medical professionals we take rightful pride in our collective ability to prop up ageing bodies. But every now and then one is faced with a patient that forces the question: what makes a person age beyond their years, and perhaps to die before their time? With temperatures routinely soaring over 41 degrees, Palakkad like many other parts of Kerala has had an excruciating summer this year. The heat affects not only those who have to go outdoors for work - including #palliativecare teams - but also wreaks havoc on those already weakened by age, illness, infirmity or plain misfortune. "..Our clothes are ripped and torn And you listen all over our naked body. As to the cause of our illness A glance at our rags would be more Revealing. One and the same cause wears out Our bodies and our clothes. The pain in our shoulder comes You say, from the damp; and this is also the cause Of the patch on the apartment wall. So tell us then: Where does the damp come from?" I was reminded of these immortal lines from Bertolt Brecht as we saw the gentleman in picture last Saturday. The dehydration is a medical problem we could fix. But what about the shelter made of tin sheets that made his corner little better than an oven? Or the fact that most of the caregiving is down to his grandchildren, a boy of 11 and a girl aged 9; themselves living in the same makeshift accommodation besides an unfinished house under construction? Reflecting on the profound Social Suffering experienced by patients with serious illnesses. Today I had an encounter with a patient having Cancer. Despite her advanced disease, our Palliative Care team stabilized her physical symptoms, enabling her discharge home. However, as her family was opposed that she leaves hospital, new symptoms emerged—confusion and memory disorders—causing distress to her family and raising concerns about discharge and the need of further investigations ( Neurology consultation, Brain CT...). After looking closely, I found out her symptoms were because she'd been in the hospital for a month and felt like she had no say in her own decisions, plus her family was being too protective. Recognizing that these symptoms showed her Social Suffering, we made sure to respect her choices, which helped her feel better. By offering her active listening and real control over her decisions, her symptoms started to go away, letting her go home without needing more tests. This experience underscores the crucial link between patient autonomy and their sense of dignity, highlighting the importance of respecting patient's wishes especially at the end of life. #Dignity #Palliativecare #Ethics

World Hospice and Palliative Care Day Logo released! World Hospice and Palliative Care Day (WHPCD) 2024 will be on 12 October. Pallium India’s advocacy for palliative care at “Koottam": The hallmark of palliative care in Kerala is participation of the community. And students form an important part of the workforce. In an effort to reach out to more students, Pallium India's social engagement team participated in “Koottam”, a carnival organized by the non-government organization “Volunteer India” at Loyola School, Trivandrum, Kerala on 05 May 2024.
